Insert / Update bug

hi,

i've noticed a very odd problem.
i have the attached transformation, yesterday one of my clients notified me that one row i missing in our data warehouse.
unfortunately he was right, i run the transformation again, and still - missing row.

using the kettle gui, in the preview i got back the missing row, but no insert was made.

what am i doing wrong? is that a bug in kettle or something is missing in my transformation or configuration?

btw, i emptied the table, and run the transformation again and everything was good, but i'm still wondering about this problem, and i'm afraid that it will happen again.
